The team was established under Yau Tong Social Centre for the Elderly in 1979. It was moved to Tak Tin Estate in 1993 due to the re-development of Yau Tong Estate. In April 2003, the team was restructured into an integrated home care service team to provide enhanced nursing care and rehabilitation services to our service users. The team is operated under a cluster-based management approach and connects with different professions so as to provide a continuous, comprehensive and high quality service to our service users which enables them to age in place with dignity.
