The support team was set up in January 2012, is a part of services coordinated by the “Kwong Wah Hospital Integrated Discharge Support Programme”, the target population of this team are the senior citizen who aged 60 or above and resident in either Yau Tsim Mong or Wong Tai Sin Districts, Transitional home care and rehabilitation services and support are provided by the team. The team provide services to reduce the unplanned hospital admissions of the discharged senior citizen, and to support and alleviate stress of their carers through trainings and services.
